Antithrombotic management in patients undergoing cardiac implantable electronic device implantation

چکیده
Cardiac implantable electronic devices (CIEDs) are an established treatment option for arrhythmias, sudden cardiac death prevention, and heart failure. Approximately 1000 implanted per million inhabitants in European countries each year. However, the main concern patients with indication CIED implantation is frequently associated comorbidities requiring antithrombotic medications. The invasive device procedure represents a bleeding risk ranging from pocket hematoma to tamponade. On other hand, temporary interruption of therapy increases thromboembolic events. Implanting CIEDs on medications incites several clinical dilemmas balancing against risk, as complications higher mortality rates both aspects. most common complication haematoma formation, which prolonged hospital stay, cost, infection, thus morbidity mortality. Studies have shown that heparin bridging strategy oral anticoagulants imposes greater formation no benefit reducing Most procedures can be performed safely uninterrupted anticoagulants. Dual antiplatelet should avoided whenever possible.

Oral anticoagulation management in patients with atrial fibrillation undergoing cardiac implantable electronic device implantation
BACKGROUND Oral anticoagulation (OAC) therapy is associated with increased periprocedural risks after cardiac implantable electronic device (CIED) implantation. Patterns of anticoagulation management involving non-vitamin K antagonist oral anticoagulants (NOACs) have not been characterized. HYPOTHESIS Anticoagulation strategies and outcomes differ by anticoagulant type in patients undergoing ...

Anticoagulants are commonly used drugs that are frequently encountered during device placement. Deciding when to halt or continue the use of anticoagulants is a balance between the risks of thromboembolism versus bleeding. Patients taking warfarin with a high risk of thromboembolism should continue to take their warfarin without interruption during device placement while ensuring their internat...

The aim of this review is to formulate practical recommendations for the management of antithrombotic therapy in patients undergoing cardiac implantable electronic device (CIED) surgery by providing indications for a systematic approach to the problem integrating general technical considerations with patient-specific elements based on a careful evaluation of the balance between haemorrhagic and...
متن کاملCardiac implantable electronic device infections: presentation, management, and patient outcomes.
BACKGROUND Indications for cardiac implantable electronic devices (CIEDs) are increasing. Although CIED infections occur infrequently, the impact of this outcome is expected to be substantial. OBJECTIVE The purpose of this study was to the evaluate the outcome of patients undergoing removal of infected CIEDs. METHODS A retrospective study was conducted of all patients with proven or suspect...
متن کاملDosimetric review of cardiac implantable electronic device patients receiving radiotherapy
A formal communication process was established and evaluated for the management of patients with cardiac implantable electronic devices (CIEDs) receiving radiation therapy (RT). Methods to estimate dose to the CIED were evaluated for their appropriateness in the management of these patients. A retrospective, institutional review board (IRB) approved study of 69 patients with CIEDs treated with ...
